[Konvas] Oct-18 mounts

I have searched the Konvas archives and found no immediate reference to my following question, so it may be valuable to discuss it here. As some of you may know, I had my first shoot last Wed with my 1-M and I learned a tremendous amount on the set. It went ok.
One recurring problem that I faced (and hoped I wouldn't, but there it was nonetheless) was the tendency for my lomo lenses to pop out of the oct-18 mounts. I had discovered this tendency weeks before, but it never occurred to me as a major problem until the day of the shoot when I had a shot that called for me to change the focal plane while the camera was running. I had to maintain a strong pressure on the lens as I manipulated the focus tabs which of course made the camera shake and who knows if the shot will be usable. The particular lens was the 50mm; though this happens with several other lenses including a Photon zoom.
Thereafter I took the turret off and inspected the locking mechanisms. The little locking "wings" all appeared to have no wear, though one side of them is intentionally beveled and seems to be the root of the problem. Are there any fixes, mods etc. that anyone can recommend? Perhaps the problem is with the lens itself, however the little detent groove on the lenses does not appear to have wear that would cause this.
I was thinking of perminantly fixing the lenses onto the turret as I have several camera bodies and turrets. This is a bit extreme though.
One thing, the focus action on some of the lenses is a bit tight, could this be the root of the problem, or ought the lenses stay in place regardless of the tightness of the focus ring?
Has anyone else had this problem?
